Address Form Data Entry and Edit Guidelines

The following table summarizes the data entry and edit requirements for each field on the Address & Phone Information Entry/Update screen. These guidelines are applicable to the Borrower, Reference and Comaker Address & Phone Information screens.

Descriptor	Optional	16 characters; alphanumeric	Should have a value of "Primary" for a Borrower Address. Should have a value of "Reference" for a Reference address. Should have a value of "Comaker" for a Comaker address. Should have a value of "Staff" for a Staff Member address.
From Date	Required	10 characters	Format must be "mm/dd/yyyy". Use the Calendar icon to populate this field with a valid value; see the Calendar Icon link for more information.
Thru Date	Optional	10 characters	Format must be "mm/dd/yyyy". Use the Calendar icon to populate this field with a valid value; see the Calendar Icon link for more information. If a value is entered, it must be greater than or equal to the From Date.
Bad Since Date	Optional	10 characters	Format must be "mm/dd/yyyy". Use the Calendar icon to populate this field with a valid value; see the Calendar Icon link for more information. If a value is entered, it must be greater than or equal to the Thru Date.
Care Of	Optional	50 characters; alphanumeric	None.
Address Line 1	Required	30 characters; alphanumeric	A value must be entered.
Address Line 2	Optional	30 characters; alphanumeric	None.
City	Required *	18 characters; alphanumeric	*Required if the Country field value is "USA"; otherwise optional entry.
State	Required *	Select value from drop-down list	*Required if Country field value is "USA"; otherwise optional entry.
Zip Code	Required *	5 or 9 numeric characters	*Required if Country field value is "USA"; otherwise optional entry. Format must be "nnnnn" or "nnnnnnnnn" or "nnnnn-nnnn".
Carrier Route	Optional	2 characters; alphanumeric	None.
Country	Required	30 characters; alphanumeric	Default value is "USA".
Email Address	Optional	50 characters; alphanumeric	None.
By Whom	Read Only	10 characters; alphanumeric	Populated by CLM application/system.
Remarks	Optional	32 characters; alphanumeric	None.
Daytime Phone	Required	10 numeric characters	Format must be "nnnnnnnnnn" or "nnn-nnn-nnnn".
Daytime Phone Extension	Optional	4 numeric characters	None.
Evening Phone	Optional	10 numeric characters	Format (if entered) must be "nnnnnnnnnn" or "nnn-nnn-nnnn".
Evening Phone Extension	Optional	4 numeric characters	None.
Fax Number	Optional	10 numeric characters	Format (if entered) must be "nnnnnnnnnn" or "nnn-nnn-nnnn".
